OPTIONAL: For attorney professional ethics rules related to conflicts of interest, we need to have client names and opposing party names clear conflicts checks before scheduling appointments. For ID Doc issues, this include both the name on current legal identity documents, and--if applicable--the name that will be on legal identity documents after the corrections process for our records. For minors or those under legal guardianship, please include applicant guardian's legal names. Please list client, any legal guardian, and any adverse party names below if you feel comfortable doing so, to speed up checks and scheduling with an attorney:
